Health Insurance Information
California State University East Bay (CSUEB) requires that F-1 & J-1 visa students purchase and maintain adequate health insurance coverage during their period of enrollment at Cal State East Bay.
CSUEB provides a preferred health insurance vendor and does not accept alternative insurance health plans.
Understanding Student Insurance Coverage
Alliant Insurance Services is a broker that connects CSUEB with insurance companies, such as Wellfleet, that offer comprehensive, affordable health insurance plans. You will purchase your health insurance plan on the Wellfleet website.

Health Insurance
To purchase insurance
- Go to:
- Select the "Enroll" icon
- On the Enroll screen, select Enroll in correct category
- Undergraduate- bachelor degree and 2nd bachelor degree students
- Graduate- master's degree and certificate students
- Global Academy Students- Intensive English, Pre-Business Analytics, Pre-Graduate Studies, and Visiting Student Program students
- You will be directed to the Student Profile page.Complete all information. Be sure to use the correct horizon email address and correct NETID.
- Incorrect information will cause a delay in your hold being moved!
- Proceed with purchase.
- CIE will automatically move your hold. During peak times (beginning of April and November) the holds will be moved multiple times a day. During non-peak times, they will be moved once a day.
F-1 students will always have a health insurance hold on their MyCSUEB account. When you purchase insurance, the hold will be moved to a future term, it will not be removed. It is important that you check the Health Insurance Hold information to see which term the hold is preventing you from enrolling.
To Move Your Health Insurance Hold
- Purchase your insurance plan
- CIE will automatically move your hold to a future semester. It will not be removed.
- During peak times (April and November) holds will be moved multiple times a day. During non-peak times, holds will be moved once a day.
Students will not receive insurance cards by mail. You will need to download a copy of your card on the site. It is important that you download, print and carry the insurance card with you at all times.

Students should finalize their study plans for the semester before requesting a refund.
Students who have filed a claim with their insurance or who submit a refund request after the refund deadline are not eligible for a refund.
In addition, refunds are only provided to students, who have withdrawn from CSUEB and did not use the health insurance plan during the period of enrollment. Students may not attend classes during the semester then request a refund for the remaining months

Insurance Exceptions
If you are receiving health insurance coverage through an employer-sponsored plan (i.e. US policy through an employer or your spouse/parent's employer sponsored insurance), provide documentation of your current insurance policy to be vetted for approval. Please note your insurance coverage must begin on or before the first day of classes for the term you are applying for. Plans that begin after the first day of classes for the term you are applying for will not be approved.
You will be required to submit this documentation every semester!
Repatriation and Medical Evacuation Insurance
The university requires all F-1 students to purchase repatriation/evacuation insurance to ensure they have the resources necessary to return to their home country in the event of a catastrophic event or to have their remains returned to their country should an accident occur resulting in a student's death. F-1 students are in non-immigrant status and are therefore subject to this requirement. You will be instructed to purchase the Medical Evacuation and Repatriation plan through Wellfleet to complete this requirement.
Plan Ahead
To Request a Waiver due to Employee-Sponsored Insurance
- Obtain an electronic copy of your Summary of Benefits and Proof of Insurance (that includes your name on the policy and eligibility dates.)
- E-mail the following to csuebhealthinsurance@csueastbay.edu.
- Your full name, NETID and Date of Birth
- Copy of both sides of your health insurance ID card
- Your full insurance policy
- Employer health insurance enrollment confirmation email
- Proof of your relationship to the primary health insurance holder if you are not the primary.
- Marriage Certificate
- Birth Certificate
- Certificate of Registration of Domestic Partnership
- The review process may take up to 3-5 business days.
- If approved, you will be required to purchase the Wellfleet's Medical Evacuation & Repatriation plan. Instructions to purchase will be provided to you upon waiver approval.
- Once you have purchased the plan, please inform csuebhealthinsurance@csueastbay.edu.
- Your hold will be moved once we receive confirmation of the Medical Evacuation and Repatriation plan purchase.
The following types of insurance will NOT be accepted for waivers:
- Travel Insurance
- Short-term medical plans
- Socialized medicine policies, including Canadian policies
- International insurance plans are underwritten in a country outside of the United States
- California Medi-Cal or out-of-state Medicaid health plans
If you are a current student who has a financial guarantee (SACM, Kuwait), you may qualify for a waiver upon submission of the FG.
- E-mail proof of your government-sponsored scholarship that includes reference to your health insurance to csuebhealthinsurance@csueastbay.edu. Include your full name and NetID in your email.
- The review process may take 3-5 days
- The hold will be moved for the period of coverage listed.
Coverage Dates for each semester
Insurance holds are permanent on your student record. Your holds will automatically be moved to a future term based on the expiration date of the insurance plan you purchased. Email csuebhealthinsurance@csueastbay.edu if you have any questions.

Dependents
F-2 dependents are strongly encouraged to purchase health insurance for their time in the USA. F-2 dependents can follow the procedures to above to purchase- make sure to indicate your visa type as "F-2"
J-2 dependents are required to have health insurance coverage.
